This screening tool uses the Patient Health Questionnaire-9 (PHQ-9), a validated instrument widely used by health professionals to assess depression severity. It asks about how you've been feeling over the past two weeks.

The PHQ-9 has 9 items rated on a 4-point scale, with a total score from 0 to 27. Scores are grouped into severity bands: minimal (0–4), mild (5–9), moderate (10–14), moderately severe (15–19) and severe (20–27). Question 9 asks about thoughts of self-harm — if you indicate anything other than 'Not at all', crisis support resources will be displayed immediately.
