Depression, Anxiety & Stress Scale (DASS-21)
This is a screening tool, not a diagnosis.
These questions are for educational purposes only. They do not replace a clinical assessment, and a formal diagnosis can only be made by a qualified health professional such as a psychiatrist. If you're concerned about your mental health, please speak with your GP or a psychiatrist.
This screening tool uses the Depression, Anxiety and Stress Scale-21 (DASS-21), a validated instrument that assesses all three emotional states simultaneously. It asks about how much each statement applied to you over the past week.
What to expect
- 21 questions, taking approximately 5 min
- Your answers are processed in your browser — they are never sent anywhere.
- You'll see your score, severity band and recommended next steps immediately.
The DASS-21 is a shorter version of the DASS-42, with 21 items divided into three subscales of 7 items each: Depression, Anxiety and Stress. Each subscale score is multiplied by 2 to match the full DASS-42 scale. Scores are then grouped into severity bands: normal, mild, moderate, severe and extremely severe. The three subscales provide a more complete picture than a single score.
